The practice management system remains the official record. ToothNote.ai combines the exam conversation with supported Perio, Endo, and OMFS findings. The same case can support a clinical note, the letter back to the referring dentist, and a personalized patient take-home with the patient's name, findings, documented plan, and next step. The clinician reviews each document before use or delivery. Practices that already use notes in their PMS or an AI scribe can start with a task their team still needs to finish. ToothNote.ai is not focused on enterprise DSOs yet. ## Six Primary Workflows ToothNote.ai keeps six daily jobs explicit rather than treating them as generic templates: - **Patient Exam:** the recorded specialist encounter becomes a reviewed SOAP note with supported case context. - **Treatment Note:** procedure documentation keeps the patient, tooth, materials, findings, and follow-up together. - **Referral Letter:** writes back to the doctor who sent the patient in, using a short memo or the reviewed encounter. - **Insurance Appeal:** connects the denial, claim context, clinical findings, and patient-bound evidence in a reviewed appeal case. - **Medical Clearance:** prepares a one-page clearance form for the patient's physician: planned-treatment checkboxes, drafted conditions and dentist comments, a fixed physician question set, and sign-and-fax return instructions. Emailed as a PDF, downloaded, or printed; no fax transmission or e-signature. - **Outbound Referral:** sends the patient to another office with two one-page documents on the practice letterhead: a patient copy with the destination, a plain-language reason, a labeled tooth chart, and a printed map of the receiving office, and an office copy with the referring doctor, patient, procedure, teeth, and notes. Each copy is emailed, downloaded, or printed on its own. Referral Letter and Outbound Referral are opposite directions. The first writes back to the doctor who sent the patient in; the second sends the patient to another office. Every document is doctor-approved before use or delivery. ## Patient Chart The Patient Chart is a working clinical view of the patient information ToothNote.ai has captured and reviewed. It brings the room-ready summary, patient-specific workflow actions, tooth-level findings, recent radiographs, clinical photos, specialty measurements, notes and letters, relationships, patient details, and clinical history into one view. Today, the practice management system remains the official chart of record. ## Doctor Chart The Doctor Chart organizes the referring-doctor relationship across patients and documents. It shows referral and active-patient counts, open-loop and letter-on-file status, recent referral activity, an AI relationship summary, contact and delivery context, patient relationships, and supported doctor-bound actions. It is a referral relationship view, not a generic CRM. ## Specialty Measurements - **Periodontics:** voice-driven and keyboard entry feed six-site charting with probing depth, recession, bleeding, suppuration, mobility, furcation, implants, missing teeth, and supported CAL or AAP outputs when the required source data exists. - **Endodontics:** a structured test grid for the evaluated tooth, control tooth, EPT values, signed comparison, cold, percussion, palpation, and bite findings. - **Oral and maxillofacial surgery:** supported InnerView NFE and device-measured mobility captures with tooth, source, session, protocol, implant, missing-site, and ERG context preserved. Measurements remain source-attributed and clinician-reviewed. ToothNote.ai preserves the finding; the clinician interprets it. The Periodontics, Endodontics, and Oral and Maxillofacial Surgery pages connect each specialty's measurements to the repeated work around the case: clinical notes, the letter back to the referring dentist, outbound referrals, patient recaps, medical clearances, and insurance appeals. They also describe the current copy, download, email, and controlled-release Open Dental delivery paths without claiming unsupported PMS write-back. ## Referral Letter Speed A 30-second voice memo can become a polished, deliverable referral letter in under two minutes, including addressing, salutation, clinical framing, and delivery by email. ## Periodontal Intelligence ToothNote.ai includes periodontist-grade perio charting: voice-driven capture, keyboard probing with auto-advance, six sites per tooth, site-level bleeding and suppuration, per-surface furcation, mobility grades, and a one-keystroke implant mode for peri-implant charting. The clinician reviews and confirms the chart before it becomes the patient-bound record. For Practice customers with tooth-level evidence enabled, doctor-confirmed six-site periodontal findings and radiograph-derived restorative findings appear together at the tooth level with source and verification cues. The doctor can review one tooth-level clinical picture instead of reconciling the exam, chart, and radiograph in separate places. Where enabled, ToothNote.ai drafts AAP 2017 stage and grade from clinician-verified periodontal charting. Staging is never drafted from unreviewed or unverified chart reads. ## Radiograph-Aware Documentation For Practice customers with radiograph findings enabled, ToothNote.ai proposes visible restorative and prior-treatment findings from radiographs, maps each candidate finding to a tooth, and shows its source and confidence for doctor confirmation. In the panoramic example shown on the homepage, it proposed crowns, post-and-core treatment, root-canal filling material, a direct filling, and an implant fixture. The referral letter and clinical note can reflect visible, source-labeled evidence instead of relying on the conversation alone. This gives oral surgery and endodontic practices richer drafts while keeping every finding subject to doctor review. ## Plans Pricing is per doctor per month. - **Solo:** $99 monthly or $79 per month billed annually. Unlimited referral letters, email delivery, Patient Chart, and Doctor Chart for up to 50 referring doctors. - **Pro:** $249 monthly or $199 per month billed annually. Exam and treatment notes, medical clearances, outbound referrals, unlimited referring-doctor records, and analytics. - **Practice:** $499 monthly or $399 per month billed annually.
